Patient Portal

Thompson Peak Family Care is innovative in its use of medical information technology. Our goal is to provide our patients with state-of-the-art medical care while making their experience as convenient and stress-free as possible. To help meet this goal we have implemented a web-based Patient Portal which will provide our patients with:

• Access to test results, medication lists, and diagnoses
• The ability to request appointments, prescription refills, and to communicate with their physician via secure messaging directly through their electronic health record
• The ability to download and complete patient information forms before their appointment
• The ability to pay outstanding balances online

Registering for the Patient Portal is a simple process:

• Visit the Patient Portal website
• Click on “register here” on the Patient Portal welcome page
• Enter your first name, last name, date of birth, and home/primary phone number, and proceed as directed
• Create your own patient identification number (PIN)
