Q. WHAT IS A FAMILY MEDICINE PHYSICIAN?
A. A Family Medicine physician is a doctor who takes care of your whole family. Family Medicine physicians create caring
relationships with patients and their families. They really get to know their patients. They listen to them and help them make the right
health care decisions. Family Medicine physicians take care of the physical and emotional health of their patients and their families.
They know how your family’s health history can affect you. They are trained to care for you through all the stages of your life.
Family Medicine physicians are trained in all areas of medicine. They diagnose and treat the full range of problems people bring to
their doctors. They know when to treat you and when to bring in another specialist you can trust. Family Medicine physicians know
the most current treatments and technologies.
Only Family Medicine physicians train for 3 years in real practice settings, treating
patients in the office, the hospital, and at home. And they re-certify more than any other medical specialty. Family Medicine
physicians also continue to educate themselves. This allows them to apply the latest medical breakthroughs to the everyday care
of their patients. Family Medicine physicians are specially trained in preventive medicine. They believe that preventing a health
problem is better than having to overcome one. They help you make the right health choices to keep you and your family healthy.
Q. What are your office hours?
A.Thompson Peak Family Care’s office hours are 7 AM to 5 PM Monday through Friday.
Q. What should I do if I need to see a doctor after hours?
A. In the case of an emergency you should call 911 or proceed to the nearest emergency room. However, in a
non-emergency situation, you may call us at 480-505-3484 and follow the directions to leave a message for the on-call physician.
We ask that calls regarding prescription refills, test results, or other non-urgent issues be dealt with by contacting the office
during regular business hours. PLEASE NOTE THAT OUR ON-CALL PHYSICIANS WILL NOT PRESCRIBE PAIN MEDICATOINS OR ANTIBIOTICS WITHOUT AN OFFICE VISIT.
Q. What if I’m late for my appointment?
A. If you are more than 15 minutes late for your appointment you may be asked to reschedule.
This is done in order to accommodate the flow of patients and to provide prompt on-time service for all of our patients.
Q. What if I can’t keep my appointment?
A. We ask that you give us a 24-hour notice if you are unable to keep your scheduled appointment.
This enables us to offer your appointment slot to another patient, allowing for easier access for all of our patients.
Repeated no-shows without adequate prior notification will result in dismissal from the Practice.

Q. How do I request a medication refill?
A. Medication refills may be requested by contacting your pharmacy and having them transmit the pertinent
information electronically to our office. This will ensure that the right medication and dosage is being requested and refilled.
Medication refills may also be requested online via our Patient Portal. Please allow 1 - 2 business days for completion of refill requests.
Expedited refill requests will be associated with an additional charge. Please note that our physicians will not prescribe pain
medications or antibiotics without an office visit.
Q. How do I get test results?
A. Results may be reviewed during an office visit, online via our Patient Portal, or through our
ResultsCall™ phone line. Your physician will instruct you how to obtain your results.
The Patient Portal and ResultsCall™ phone line are available to you at your convenience,
24 hours a day seven days a week.
You will receive a telephone message and an e-mail notification that your results are ready to
be retrieved. It is very important that you notify us with any changes in your preferred phone number or e-mail address as
this will affect our ability to contact you. If you would like to have your results sent to an alternate phone number or e-mail address,
please notify the office so we can make the appropriate change.
To retrieve your results by phone, dial 1-877-269-8885 and follow
the simple instructions. You will need your PIN to access the ResultsCall™ delivery system
(this is the number that was created for you at the office or you created online while registering for the Patient Portal).
Please listen to the entire message carefully to ensure that you receive all the information and instructions left for you.

Q. What is a patient centered medical home?
A. The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. A patient-centered medical home integrates patients as active participants in their own health and well-being. Patients are cared for by a physician who leads a medical team all working together to coordinate the patient’s care. In addition to your personal physician, this team may include advanced practice nurses, pharmacists, nutritionists, social workers, educators, and care coordinators. These relationships offer patients comfort, convenience, and optimal health throughout their lifetimes. The primary care medical home focuses on prevention and wellness, and a comprehensive team approach to care. Coordination of care and superb access to care matching the patients’ needs and preferences is a priority.
