WHAT TO BRING TO YOUR APPOINTMENT

The following is a list of a few important items you will need to bring with you to your appointment.

• Your insurance card.
• A list of current prescription medications, herbal supplements, vitamins, and/or over-the-counter medications you are taking, including dose and frequency. This will allow your physician to check for potentially dangerous drug interactions using our advanced electronic medical record software.
• Pertinent information about your medical and surgical history.
• Names and addresses of previous physicians and, if possible, please have your previous medical records sent to our office prior to your visit.

CANCELLATIONS

Should you need to cancel or reschedule your appointment, we do request a 24-hour notice. Repeated no-shows without adequate prior notification will result in dismissal from the Practice.
